The Neighborhood Advisor Program can provide support to senior citizens ages 60 and above who live in the Baldwinsville area and the Town of Elbridge. This includes the Villages of Baldwinsville, Elbridge, and Jordan and the Towns of Lysander, Van Buren, and Elbridge.

The Neighborhood Advisor Program provides services to seniors, especially isolated and homebound seniors, who need assistance in order to remain independent and living in their own homes.

The Neighborhood Advisor Program works to assess an individual’s needs and then helps to connect that senior with resources to serve those needs.

The Neighborhood Advisor Program can provide assistance with a wide variety of concerns, such as:

  • Assistance with completing and submitting applications and/or recertification for:
    • New York State H.E.A.P.
    • Food Stamps
    • Medicare
    • Medicaid
    • SSI
    • LifeLine
  • Information & referrals to:
    • Weatherization/Home Repair
    • Senior Nutrition & Meals on Wheels
    • Legal Assistance
    • Transportation
    • Health Services
    • Social Programs

The Neighborhood Advisor Program is a free service to the community.

EISEP (Expanded In-Home Services for the Elderly Program)

Expanded In-Home Services for the Elderly Program (EISEP) provides case management for seniors that include in-home personal care, light housekeeping, laundry, and meal prep for non-Medicaid eligible seniors.

  • The fee goes according to income. There is no fee for Case Management services.
  • Eligibility:-Individuals within the listed areas:  Baldwinsville, Lysander, Phoenix, Memphis, Liverpool, and Cicero-Senior must be at least 60 years old and need assistance with personal care, housekeeping, laundry, or meal prep (and is not eligible for Medicaid)-If interested please call Onondaga County Office of Aging at (315) 435-3355 ex 4943 or ex 4693
  • Please contact Onondaga County to be referred as a Case Manager cannot take direct referrals
  • Other services may be available depending on circumstance and available funding
